General

What are your hours of operation?

We are open Monday – Thursday 8AM-7PM, Friday 8AM-4PM, and Saturday 9AM-1PM

What insurance policies do you accept?

We participate in Medicare and most HMO, PPO, and traditional indemnity insurance plans.  Please check with your insurance company to verify we participate with your particular plan.

What hospitals do you admit patients to?

We admit patients to Northside Hospital Forsyth in Cumming, Georgia and Emory John’s Creek Hospital in John’s Creek, Georgia

What is a Nurse Practitioner?

A Nurse Practitioner is a Registered Nurse who has earned a Masters Degree in Nursing.  They are clinically trained to see patients as an independent caregiver.

Do you use Nurse Practitioners?

Yes we have several Board Certified Nurse Practitioners.

Appointments

How long does it take to get an appointment?

We offer same day sick visit appointments and have generous availability for Physicals and Wellness visits.

What do I need to bring with me to my first visit?

Please bring the following items with you:

  • Insurance card
  • A valid photo I.D.
  • All medications you are currently taking in the bottle
  • List of any allergies
  • Medical records and immunization records
  • Your pharmacy’s name, location, and phone number

Please arrive at least 30 minutes before tour appointment time so you have time to complete your new patient paperwork.

What do I need to bring with me to any subsequent visits?

Please bring the following items with you:

  • Insurance card
  • A valid photo I.D.
  • All medications you are currently taking in the bottle
  • List of any allergies


Please arrive at least 15 minutes before your appointment time.

We are gladly accepting new patients into our Practice.  Please call out office at 770-945-1990 to schedule an appointment today.

Billing

Who do I call if I have a question about my bill?

Our billing department is available to take your calls Monday through Friday from 8:00 AM to 4:00 PM.  Please call 770-945-1990, EXT 232.

Should you have a question about an invoice from Quest or LabCorp, please call our Insurance Department at 770-945-1990, EXT 230.

What forms of payment are accepted?

We gladly accept cash, check, Discover, MasterCard, and Visa

What do I do if I need a refill?

Please refer to our Prescription Policy under the Forms section.
